Your First Appointment

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

• Patient's insurance card
• List of current medication as well as any vitamins and/or supplements you are taking (or bring in your medications)
• Information about patient's medical and surgical history
• Recent test results, labs, x-rays, or relevant records
• Authorization from insurance company (the referral)
• Completed patient forms (PDF)

*Please try to come to the office with a comfortably, but not excessively, full bladder in case a urine specimen is needed.

*For safety reasons, please discontinue all vitamins, minerals, herbs, and supplements two weeks before surgery.

Insurance and Payment Information

Dr. Gordon is a provider for Medicare and most major insurance plans, including Aetna, Arizona Foundation, Blue Cross Blue Shield, Beech Street, CMDP, First Health, Great West, Humana, Healthnet, Mayo, Pacificare Direct, PHCS, Schaller Anderson, and United Healthcare and provides insurance billing. You are responsible for co-payments, deductibles, or charges that are not covered by your insurance at the time of service. If you have questions regarding billing or which insurance plans we accept, please contact us. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.

Privacy Practices

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
